A/C crashed during training flight in a NG training area near Dragoon Creek at approx. 1530 hours. Crew was performing a transition training flight and was on final approach when IP initiated a simulated single-engine emergency procedure by beeping the #2 engine offline. As A/C reached short final, approach was lower and slower than normal causing slingload to hit trees. CE tried to release load but load hit ground and caused A/C M/R blades to enter treeline, severed M/R blade hit a T/R blade causing T/R gearbox assy to separate. A/C came to rest with the main fuselage upright and tailboom lying across slingload. |

The A/C crashed in the center of a large building complex known as the Ditto Technical Area. A/C was being used to perform experimental work in which E158 CS (tear gas/riot control agent) canister cluster were being dropped. One of the clusters onboard the A/C malfunctioned, setting the helicopter on fire and filling its interior with CS. |

A/C was on a scheduled VIP flight and after departure, at 250' AGL, one blade became unlocked from the horn pin, started to flap and tumble, and tore loose from the head. A/C started to come apart in the air, nosed over, crashed to the ground and burst into flames approx 23 mi from Parsberg. Yellow blade came loose in flight and struck fuselage near upper left pylon causing progressive disintegration of acft. Cause was due to horn lock assy only partially engaged in the sleeve spindle assy.
BG Spengler was the commander of the 32nd Air Defense Brigade and COL Light was the commander of the 10th Air Defense Group. |

A/C impacted in an approx 35 deg nose-low attitude and 15 deg left bank and traveled approx 35 ft after impact coming to rest on left side. Investigation determined that the ASE was engaged shortly after takeoff at an altiude of 100-150', and the ASE motor control override switch was in the Left-Forward-Down position which caused acft to be placed in an attitude from which the pilot could not recover prior to impact. |

impacting the ground on its left side of cockpit, forward of copilot's seat. *Pilot died one day later of injuries. |

Three helicopters crashed east of Red Bluff, became disoriented in a blanket of fog and flew into a rocky hill. Two of the aircraft crashed while the third crew suffered only injuries. The flight was only a part of a flight of 19 heading from Ft Riley to Ft Lewis. |

A/C crashed north of Yong Wol after running into bad weather while enroute to the aid of a seriously injured American civilian construction worker. As A/C neared the the destination point at night, altitude was lowered and landing light and search light was sweeping the area when A/C struck a 7-strand electrical line, M/R blade severed the tailboom, struck more wires, somersaulted and dropped vertically to the ground. |

Pilot attempted to land in open field but aft rotor blades struck a tree resulting in nose-high attitude, aft blades struck second tree and a third tree sheared horizontal stabilizer. A/C began disintegrating and hit ground coming to rest on its side. |

A/C crashed into a rain-made desert lake approx 15 mi NE of White Sands Proving Ground HQ near Alamogordo while on a routine flight from Ft Huachuca during a violent storm's downdraft. After encountering heavy showers, A/C was observed to be following highway, made a 180 deg turn and flew back into a lighter area, turned again and flew back into storm. Short time later A/C crashed in a nose-down attitude, disintegrated upon initial impact. Mechanic was thrown from A/C. |

Mid-air with an 11 ABN H-13 about 75' AGL over an open area at Flak Casern near Augsburg. Choctaw observed other A/C land and PAX depart then diverted attention to clearing poles on approach. OH-13 proceeded to takeoff unaware of H-34 landing. M/R blades from both meshed and OH-13 began to burn and fell inverted to the ground. H-34 hung momentarily and then dropped to the ground and came to rest on left side. One ground soldier suffered a severed leg from debris coming from aircraft. |

A/C departed on a 2-hr night training mission and shortly after takeoff, over a heavily-populated area, A/C was observed at about 600' AGL, executing a right turn when the nose pitched up, followed by chopping off the tail pylon. A/C rolled sharply right, with the nose still high and one of the rotor blades separated from the hub. A/C began to disintegrate as the remaining blades progressively chopped the tail cone from the rear forward, crashed and burned in a heavily wooded area about a half mile from a housing development near Ft Benning. Investigation opinion was that a blade stall occurred. |

A/C were conducting night flight training making landings to flashlight signals given by a ground control party on unlighted landing sites near Schwieberdingen. At approx 200' AGL, crew turned on landing light and beam was diffused by haze. Landing light was switched back off and appeared that crew was contemplating a go-around. Shortly after passing the ground control, A/C assumed a shallow diving attitude, increased speed and struck the ground approx 900' beyond intended landing point and burst into flames. |
